Any device that is intended to be used for medical purposes within the home.
Includes orthotics, prosthetics, and supplies. All requestors must be referred by a healthcare provider or organization and have a written order or otherwise recommendation from a physician for the needed equipment. Insurance information shall be provided and reviewed to ensure that the requestor has an immediate financial need.
In some cases where insurance covers a portion of the cost, Children’s Adaptability Network may help with out-of-pocket expenses with the goal of easing any related financial burden to the family or caregiver.
Includes any medical service which is not directly provided by the patient’s physician but is recommended to:
(a.) obtain a proper diagnosis for the patient,
(b.) assist in the patient’s short or long-term development, or
(c.) improve mobility for the patient.
Examples include (but are not limited to) laboratory diagnostics, physical, occupational, or speech therapies, or home health services. Transportation services may also be provided.
All requestors must be referred by a healthcare provider or organization and have a written order or otherwise recommendation from a physician for all ancillary services. Parents or caregivers also have the right to provide a letter of justification, which will then be reviewed by the board for approval.
Infants and toddlers (aged 0-36 months) will be referred to the Virginia Department of Behavioral Health and Developmental Service’s Early Intervention Program for therapeutic services.
Described as any item that is justified as a daily living aid or that allows the child to reach his or her highest potential.
These items may also include duplicate DME which makes day-to-day care easier for the family or caregiver. These items constitute a wide range of products including (but not limited to):
No medical referral or insurance information is needed for items that are requested to improve the overall quality of life of a child. The requestor should fill out the QOL request form with a short description of the needed item and a brief statement on how this item will aid the child.
Children’s Adaptability Network provides proper guidance and additional resources to families based on their specific needs. Resources may include educational materials, guidance in navigating state Medicaid waiver processes, or referral to partnering organizations that may
otherwise be able to assist the family in some manner.
